This form requires digital signature with 'smart card' which includes certified certificate. This signature is admissible by law.
הערות:
לביצוע החתימה נדרש לחבר כרטיס חכם למחשב באמצעות קורא כרטיסים ולהשאירו מחובר במשך כל התהליך.
בסיום החתימה מתקבל קובץ PDF חתום אשר מצורף אוטומטית לטופס לצורך המשך התהליך.
ניתן לשמור את קובץ הPDF החתום בנפרד לצרכים נוספים.
החתימה על קובץ הPDF נעשית מתוך הדפדפן באמצעות שירות הטפסים ולא מתוכנת הצפייה בקובץ הPDF.

Remarks:
To sign the form you required to connect the smart card to the computer with smart card reader.
The process will create a PDF file which will be attached automatically to the form.
You can save the PDF file on your computer for further use.
The signing procedure is being processed through the browser and not through the PDF software.
